How does the program work?
After you sign up or are referred, we schedule an initial assessment visit with a registered nurse or licensed social worker. From there, you’ll receive regular home visits — up to nine during pregnancy and the postpartum period, and additional visits after your baby is born — based on your family’s needs.
How do I sign up or schedule a visit?
You can sign up directly through our website, ask a doctor or other professional to refer you, or have a friend or family member submit a referral on your behalf. Once we receive your information, we’ll contact you to schedule your first visit.
Who will be visiting me?
Visits are conducted by a registered nurse or licensed social worker, each certified through Michigan’s MIHP program and holding at least one year of relevant clinical or family services experience. Specialists such as lactation consultants, infant mental health specialists, and registered dietitians may also be part of your care team.
Is there a cost? Do I qualify?
No — all services are free. You qualify if you’re pregnant, or have an infant under 1 year old, and are enrolled in or eligible for Medicaid.
How is this different from my doctor or OB visits?
This program doesn’t replace your doctor or OB — it works alongside them. Your care team comes to you between medical appointments to provide education, benefits navigation, and connections to local resources.
